A. ESTABLISH REASON FOR VISIT AND DOCUMENT PATIENT DESCRIPTION OF CHIEF COMPLAINT. DOCUMENT ALL EPIC VISIT ENCOUNTER FIELDS AND SMARTPHRASES.

B. PARTICIPATE IN QUALITY IMPROVEMENT AND WORK FLOW ENHANCEMENTS THAT SUPPORT PATIENT CARE.

C. TAKE AND DOCUMENT PERTINENT VITAL SIGNS INCLUDING BLOOD PRESSURE, PULSE, RESPIRATIONS, HEIGHT, WEIGHT, BMI AND PAIN LEVEL.

D. ASSURE ALL REQUIRED BLOOD WORK AND Back Office DIAGNOSTIC LABS ARE COMPLETED AND DOCUMENTED IN THE EMR BEFORE PROVIDER EXAM ROOM VISIT.

E. ASSURE PROVISION OF ALL INSTRUMENTS, ORDER LABS AND VACCINES AND DOCUMENT FOR PHYSICALS AND WELL CHILD VISITS.

F. FOLLOW-UP ON PROVIDER POST EXAM REQUESTS; DRESS WOUNDS, PERFORM EAR IRRIGATIONS, RECHECK BLOOD PRESSURE ETC.

G. ADMINISTER MEDICATIONS/IMMUNIZATIONS, EVALUATE EFFECTIVENESS. ASSURE PATIENT IS INFORMED ABOUT POSSIBLE EFFECTS.

H. MAINTAIN PATIENT FLOW FOR PROVIDERS, FILLING ROOMS PER DESIGNATED QUOTA.

I. MAINTAIN QUALITY CONTROLS MEASURES DAILY.

J. CLEAN AND STOCK ROOMS PER ESTABLISHED REQUIREMENTS; CLEAN AUTOCLAVE AND STERILE INSTRUMENTS.

K. ORDER SUPPLIES, IMMUNIZATIONS AND PROVIDE ASSISTANCE AS DIRECTED OR NEEDED.

L. SERVE AS A MEMBER OF A PATIENT CENTERED MEDICAL HOME TEAM.

Provides ongoing emotional, physical and educational support before, during and after birth to families expecting a baby.

Serves as part of the Midwifery health care team.

A. Prepares the mother for birth by working with her to develop birthing plan, educating her about the birthing process, providing her with emotional support and offering support services before, during and after childbirth.

B. Works in conjunction with midwifery team to provide doula support for NFP patients. This includes patient home visits, attending Centering pregnancy/parenting with the patient, accompanying the patient during prenatal medical appointments and labor, and providing support postpartum.

C. Provides emotional support and assistance with positioning and comfort measures during childbirth.

D. Offers evidence based information to educate expecting families about pregnancy, birth, breastfeeding, and parenting one-on-one.

E. Through collaboration and outreach initiatives, develops partnerships within the community to implement and deliver high quality services.

F. Participates in patient recruitment and retention initiatives.

G. Participates in program fundraising initiatives.

H. Develops/maintains program/patient database information as required.

A. Collaborate with members of the health care team and other health professionals as it relates to essential job duties.

B. Complies with patient confidentiality and HIPAA regulations.

C. Maintains organizational skills and responsibilities in keeping appointments with patients.

D. Perform other functional related duties as assigned.

3. QUALIFICATIONS:

Participate in educational activities.

At times may be required to call patients and confirm appointments.

Handles faxes and copying as requested.

Forward all necessary correspondence and mail to Ridge or Tremont location in a timely manner.

Inventory and order office supplies.

Complete daily reconciliation report and bank deposits.

Keep office Outlook Calendar up to date.

Assist patient in completing MHCP applications.

Ensure lobby is neat and orderly.

Complete all other duties as assigned.

The Major Gifts Officer will create and implement individual and major gift strategies for short and long-term fundraising initiatives, and to strategically manage relationships in order to lead donors to increasing levels of commitment and support.

The successful candidate will demonstrate high energy and enthusiasm as well as a donor-centric approach to relationship building, always exhibiting patience, persistence and professionalism. S/he will manage all prospect contacts to ensure positive and purposeful prospect and donor relations.

The successful candidate will have extensive familiarity with fundraising to include superior knowledge of all aspects of fundraising, have exemplary communication skills with varying interactions and have the ability to work through challenges and overcome rejections.

The successful candidate will also have demonstrated fundraising, management and leadership skills preferably in the Cleveland/NEO region.

Major Responsibilities

Manages an active portfolio of 150 major gift prospects, strategically advancing prospects through the solicitation cycle and successfully closing major gifts of $10,000 or more to support TCM.

Responsible for developing strategies specifically tailored to individual prospects to move them through the solicitation cycle in and efficient and effective manner.

Responsible for meeting established donor/prospect visits, solicitation/proposal and revenue goals.

Responsible for keeping accurate and timely records of all constituent interaction in Talisma. Responsible for preparing personalized correspondence in a timely manner.

Provides regular reporting of major gift progress including detailed contact reports of prospective donor visits and submits reports to the CEO at intervals deemed appropriate by all parties.

Responsible for becoming knowledgeable about TCM and its priorities in order to communicate those proprieties effectively to prospects.

Regularly monitor and oversee gifts processing and donor benefits fulfillment to assure accuracy.

Professional Qualifications

A thorough knowledge of and proven success in securing major and planned gifts.

Demonstrated fundraising, management and leadership skills preferably in the Cleveland/NEO community.

Bachelor's degree with a major in Business or a related field.

Certified Fund Raising Executive (CFRE) preferred. The Certified Fund Raising Executive (CFRE) credential demonstrates an individual's mastery of the standards set for core knowledge and skills required of fundraising executives after five years of experience.

A minimum of three to five years' experience with individual and major gift solicitation, preferably with organizations with annual budgets of at least $4MM.

Excellent written and verbal communication skills.

Ease/ability to speak before groups of varying sizes and compositions.

Excellent planning and organizational skills. Ability to multi-task, prioritize and work well under pressure.

Self-initiator and functions well within a team environment.
